MOBILE MESSAGING PROGRAM TERMS & CONDITIONS

The Migraine Tracker is a text message (SMS) based tracking program for patients with episodic or chronic migraine. The program has been designed to help you track the frequency of your migraine days for one year. It is not intended to be a source of medical advice or care. Please contact your healthcare provider if you have any questions about your medical condition, diagnosis, treatment, or care.

The Migraine Tracker short message service (SMS) symptom tracking program (the ‘Service’) runs on the short code 43109. You can cancel the SMS service at any time by texting "STOP" to 43109, after which you receive an SMS message to confirm that you have been unsubscribed and will no longer receive SMS messages.

For assistance, you can reply “HELP” to get the customer support phone number or directly call 833-246-6844.

Carriers are not liable for delayed or undelivered messages. Message and data rates may apply. Message frequency will vary per month. If you have any questions about your message plan or data plan, contact your wireless service provider.

Amgen Inc. (“Amgen”) reserves the right, in its sole discretion, to change, modify, add, or remove these Mobile Terms of Use (“Mobile Terms”) at any time. Amgen may in its discretion change or suspend the Service (defined below) at any time. If you are dissatisfied with the Service or the content received through the Service, your sole remedy is to discontinue use of the Service.
